CAMP HORIZON

Welcome to Camp Horizon, our expansive 3-acre sanctuary in Harford County! This unique year-round day camp combines the joy of outdoor activities with the warmth of a close-knit community.

Our participants enjoy:

Camp Horizon offers a safe, inclusive environment where every individual can thrive, make new friends, and build independence. Our program runs 3 days weekly during fall and expands to 5 days during summer months.

WORK-BASED LEARNING AND EMPLOYMENT OPPORTUNITIES

At A New Horizon Inc, we create real-world employment opportunities through our own business ventures and community partnerships. Our organization proudly owns and operates: – A commercial cleaning business – A full-service print shop Plus, we maintain strong partnerships with various employers throughout the community. These diverse business operations allow us to provide authentic work-based learning experiences and meaningful employment opportunities for the individuals we serve. Through hands-on training in our businesses and partner workplaces, participants gain valuable skills and experience in real workplace settings. Our employment programs focus on developing practical job skills while fostering independence and professional growth.

Comprehensive Services

In-Home Supports

In-Home Supports are services that provide retention or improvement in skills related to activities of daily living that are necessary to enable the person to reside successfully at home in his or her community and participate in community activities based upon what is important to and for the person, as documented in his or her Individual Support Plan (ISP) and reflected in his or her Person-Centered Thinking and Discovery tools.

a. Services may be provided to people in the home or community, with the place of residence as the primary setting. A person may receive in-home supports services when his or her place of residence is his or her own home, a family home, a friend’s home, or transitional housing.

b. Services may be provided in person, by phone, or by any other technology device that supports the use of video-audio communication, such as Skype, FaceTime, etc., as approved by the person and his or her support team and documented in the ISP. In-home supports services using technology to communicate with the person shall not exceed twenty (20) percent of the total hours of in-home supports services that the person receives each week.

c. In-home supports services shall only be provided for up to eight (8) hours per day unless there is a temporary emergency. In the event of a temporary emergency, DDS may authorize up to sixteen (16) hours per day for up to one hundred and eighty (180) days, during the person’s ISP year.

In-home supports shall include the following:

a. Training and support in activities of daily living and independent living skills.
b. Support to enhance opportunities for meaningful adult activities and skills acquisition that support community integration and a person’s independence, including management of financial and personal affairs and awareness of health and safety precautions.
c. Support to enhance opportunities for community exploration aimed at the discovery of new and emerging interests and preferences, including activities aimed at supporting the person to have one or more new relationships.
d. Support to build community membership.
e. Training on, and assistance in the monitoring of health, nutrition, and physical wellness.
f. Implementation of a home therapy program under the direction of a licensed clinician.
g. Training and support to coordinate or manage tasks outlined in the Health Care Management Plan, if applicable.
h. Assistance in performing personal care, household, and homemaking tasks that are specific to the needs of the person, except that this may not comprise the entirety of the service.
i. Assistance with developing the skills necessary to reduce or eliminate behavioral episodes by implementing a Behavioral Support Plan (BSP) or positive strategies.
j. Opportunities for the person to seek employment and vocational supports to work in the community in a competitive and integrated setting.
k. Assistance with the acquisition of new skills or maintenance of existing skills based on individualized preferences and goals identified in the In-Home Supports Plan, ISP, and Plan of Care.
l. Coordinating transportation to participate in community events consistent with this service.

Supported Employment Services

Supported Employment Services are designed to provide opportunities for persons with disabilities to obtain competitive work in integrated work settings, at minimum wage or higher, and at a rate comparable to workers without disabilities performing the same tasks.

Small group supported employment services will enable the person to become part of a competitive, integrated work setting. Services and training activities are provided in regular business, industry, or community settings for a group of two (2) to four (4) workers.

Eligibility for Supported Employment Services
To be eligible for supported employment services, the person receiving services shall:
a. Be interested in obtaining full-time or part-time employment in an integrated work setting; and
b. Demonstrate that a previous application for the District of Columbia Rehabilitation Services Administration (RSA) funded supported employment services was made, by the submission of a letter documenting either:

  • Ineligibility for RSA services; or
  • The completion of RSA services with the recommendation for long-term employment support.

Supported Employment Services Shall:
a. Provide opportunities for persons with disabilities to achieve successful integrated employment consistent with the person’s goals;
b. Be recommended by the person’s Support Team; and
c. Be identified in the person’s Individual Support Plan (ISP), Plan of Care, and Summary of Supports.


Supported Employment Service Models
Supported employment services will provide the following three (3) models:

  1. Individual Job Support Model

    • Evaluates the needs of the person.
    • Places the person into an integrated competitive or customized work environment through a job discovery process.
  2. Small Group Supported Employment Model

    • Utilizes training activities for groups of two (2) to four (4) workers with disabilities.
    • Places persons in an integrated community-based work setting.
  3. Entrepreneurial Model

    • Utilizes training techniques to develop ongoing support for a small business that is owned and operated by the person.

Entrepreneurial Model Activities
Supported employment services for the entrepreneurial model shall include the following activities:
a. Assisting the person with identifying potential business opportunities.
b. Assisting the person with developing and launching a business.
c. Identifying the supports necessary for the person to operate the business.
d. Providing ongoing assistance, counseling, and guidance once the business has been launched.


Supported Employment Services Activities
Supported employment services shall consist of the following activities:
a. Intake and assessment.
b. Job placement and development.
c. Job training and support.
d. Long-term follow-along services.


Supported Employment Small Group Services Activities
Supported employment small group services shall consist of the following activities:
a. Job placement and development.
b. Job training and support.
c. Long-term follow-along services.

Employment Readiness

Employment readiness services shall be provided over a defined period of time with specific outcomes to be achieved. These services offer learning and work experiences, including volunteer work, where a person enrolled in the Waiver can develop general, non-job-task-specific strengths and new employment-related skills that improve eligibility for paid employment in an integrated community setting.

A person receiving employment readiness services may pursue employment opportunities at any time to enter the general workforce.

Staffing Ratio
Employment readiness services shall maintain the required staff-to-person ratio, as indicated in the person’s ISP and Plan of Care, with a maximum staffing ratio of 1:4.

Service Eligibility for Employment Readiness Services
To be eligible for employment readiness services, the following criteria must be met:
a. Services shall be identified in the ISP, Plan of Care, and Summary of Supports.
b. The person must demonstrate a need for employment readiness services.
c. Employment-related goals must be included in the ISP.

Employment Readiness Services Requirements
Employment readiness services shall support a person on their pathway to competitive, integrated employment and shall consist of the following:

Social and Soft Skills Training
Including, but not limited to:
a. Interpersonal skills, including building and maintaining relationships.
b. Communication skills for interacting with supervisors, coworkers, and customers.
c. Travel skills.
d. Respecting the rights of others and understanding personal rights and responsibilities.
e. Decision-making skills and strategies.
f. Support for self-determination and self-advocacy.
g. Budgeting and money management.

Developing Work Skills
Including, at a minimum, teaching the person the following:
h. Appropriate workplace attire, attitude, and conduct.
i. Work ethics.
j. Attendance and punctuality.
k. Task completion.
l. Job safety.
m. Attending to personal needs, such as personal hygiene or medication management.
n. Interviewing skills.

Additional Employment Readiness Activities
a. Coordinating transportation to community activities utilizing the Medicaid Non-Emergency Transportation Broker.
b. Employment exploration and/or employment preparation in the community.
c. Coordinating community-based, integrated, and volunteer experiences.

Community Living

Community Living includes group homes, shared living, and supported living arrangements that foster independence, community integration, and a sense of belonging. Our staff provides personalized assistance according to each individual’s ISP.
